Emergency and future healthcare preparation doesn’t change what happens. It changes how it feels when you go through it.

Without clear guidance, people rely on:

  • emotion

  • incomplete information.

That’s what makes difficult situations harder.

A Simple, Supportive Process

Two conversations. One clear document.

  • Tell me what brings you to emergency and future healthcare preparation, and your scheduling preferences so we can set up your first session. Includes your Session 1 payment to secure your booking.

  • We'll explore your values, roles, relationships, and what "quality of life" means to you. No medical decisions required — just honest conversation.

  • After Session 1 I'll prepare an initial draft of your document so you can see how what you've told me translates into a document written in your own voice. You're welcome to provide feedback at this stage — any requested changes will be incorporated into the next draft, shared after Session 2.

  • You'll complete two short workbooks (20 minutes each), done at your own pace, to help clarify your thoughts on medical interventions and real-life medical scenarios. These are thinking tools, not decisions or tests.

  • A short form to share any feedback on the first draft and confirm you'd like to continue to Session 2. Includes your Session 2 payment — choose virtual or in-person, or switch formats if you'd like to try something different.

  • We'll review your workbook reflections together and translate your values into clear guidance for the people who may one day need to speak for you.

  • Tell me what information you'd like included in your document, including contact details for your chosen Substitute Decision-Maker(s).

  • Within 7–10 business days of your Post-Session 2 form submission, you'll receive an updated draft of your personalised document — capturing your wishes in your own voice.

    You'll have one week to review it and return any feedback. I'll then refine the document — making as many changes as needed — until you're completely happy with it and ready to share it with the people who matter most. This revision process runs for up to 30 days.

  • Approximately 30 days after your document is signed off, we'll schedule a brief call — a chance to reflect, ask questions, and make any final changes before your document is considered complete.

  • If something significant changes within your first year — a new Substitute Decision-Maker, updated care wishes, or a change in your health situation — just get in touch. One complimentary round of revisions is included within the first year to keep your document current.

  • Life changes. Your plan can too. Each year I'll send you a reminder to consider whether your document still reflects your wishes. A formal annual review and update is available as a paid service if you'd like one.

From start to finish, most clients complete the process in 4–6 weeks:

  • Week 1: Complete intake form, schedule Session 1

  • Weeks 2–3: Session 1, review first draft, complete workbooks

  • Week 4: Session 2, submit Post-Session 2 form

  • Weeks 5–6: Receive and refine your document

  • Week 8–10: 30-day check-in call

This service is not medical or legal advice, and it doesn’t replace conversations with your healthcare team — it helps prepare you for them.


Optional Add-On Sessions

Once your Emergency & Future Healthcare Preparation Document is complete, you can book additional guided sessions:

SDM Session: Walk through your document with your Substitute Decision-Maker(s). I'll facilitate the conversation, answer their questions, and help them understand their role. 60 minutes, virtual or in-person.

End-of-Life Planning Session: This session focuses on what emergency and future healthcare preparation doesn't: dying and death. We explore your wishes for:

  • Body disposition (burial, cremation, donation)

  • Funeral or memorial preferences

  • Rituals that matter to you

  • Who should be involved

  • What you want people to know

This is separate from emergency and future healthcare preparation because some people need to address healthcare decisions first — without the weight of death planning in the same conversation. When you're ready, this session gives you space to think through what happens after.

  • Your Personalized Emergency & Future Healthcare Preparation Document (see example below). A clear, compassionate guide that captures your values, priorities, and wishes —written in your voice, ready to share. This becomes the reference point for anyone making health and personal care decisions on your behalf.

  • Unlimited Revisions to Sign-Off Once your document is drafted, we work together until you're completely happy with it — making as many changes as needed within a 30-day window after Session 2.

  • Optional SDM Session. Book a facilitated conversation with your Substitute Decision-Maker(s) to walk through your document together. They get clarity on their role. You get peace of mind. Available as an add-on.

  • First-Year Revision Support If something significant changes within your first year — a new Substitute Decision-Maker, updated care wishes, or a change in your health situation — one complimentary round of revisions is included to keep your document current.

  • Annual Reminder Each year you'll receive a reminder to consider whether your document still reflects your wishes. A formal annual review and update is available as a paid service if you'd like one.

  • Optional End-of-Life Planning Session This session focuses on what the core preparation process doesn't: death, dying, and what happens to your body.

  • Compassionate, Expert Guidance. Every step of the way, you’ll work with someone who understands different sides of the healthcare system — as a patient, family member, and volunteer.

Complete Emergency & Future Healthcare Preparation: $700 (virtual) or $850 (in-person)

How it works:

You pay per session.

Two sessions complete your emergency and future healthcare preparation.

Payment 1 secures Session 1.

Payment 2 is collected before Session 2.

You can pause anytime. If you don't continue to Session 2, you won't be charged the second payment.

Virtual Session (Ontario-wide): $350

  • 90-minute guided session via Google Meet

  • Available to Ontario residents only

In-Person Session (Hamilton only): $425

  • Delivered at a mutually agreed location in Hamilton (max 30 minutes travel each way)

  • Options: your home, private library room, quiet outdoor spot, or walking conversation

  • Wireless microphones capture the conversation clearly wherever we meet.
