Hi, I’m Jemima
Why I Started Sorted Planning
I know what it's like to face serious illness without having these conversations first, and I wish I’d done my own Advance Care Planning sooner.
In my late 30s, I was diagnosed with breast cancer —an experience that fundamentally shifted how I think about healthcare planning. Suddenly, I was the patient—navigating treatment decisions, side effects, and uncertainty about the future. I wished I'd thought more clearly about what mattered most to me before I was in the thick of it.
The following year, my dad faced serious illness, surgery, and time in intensive care. I watched my family struggle to make decisions on his behalf—trying to guess what he would want, wishing we'd talked about it sooner.
Those experiences changed me.
So I decided to create a space where people could have these conversations earlier—before a crisis, before a diagnosis, but most importantly, before it's too late.
My Approach
I believe Advance Care Planning should feel empowering, not intimidating.
I don't rush you through a checklist or ask you to make medical decisions you're not ready for. Instead:
I meet you where you are — whether you’re certain or uncertain
I listen more than I talk — this is about your values, not my opinions
I don’t rush you through decisions you’re not ready to make
I translate your thoughts into clear, compassionate language
I stay with you through the entire process
I create space for emotion — tears, laughter, and everything in between.
I've studied grief and bereavement, researched ACP in different countries, and spent over 14 years working in organizational planning and change management. I understand how to guide people through complex, emotional decisions with clarity and structure.
But more than that, I've lived this. I know what it's like to be the patient, the decision-maker, and supporter.

What Makes Sorted Planning Different
1. You're not too young. I work specifically with people who consider themselves “too young” because this is when planning matters most—before a health crisis forces the conversation.
2. This is about values, not just medical decisions. Our sessions focus on what makes life meaningful to you, as well as hypothetical treatment scenarios.
3. Your document is personalized—not a template. Your ACP Summary is written in your voice and reflects your unique priorities, not generic checkboxes.
4. You can take your time. There's no rush. You can pause, reflect, and return when you're ready. This process adapts to you.
